Well, in order to determine useful results, you first of all need to record these values for some time (at least half a year). Do you already have a bug tracking application that contains these information? If so, the vendor probably can give you more information about how to query the bug database for reports.

Otherwise you need to start using a bug tracking application. We use TestTrack (http://www.seapine.com). While it's not the cheapest product on the market, it's constantly enhanced. On advantage (IMO) is that it stores everything in DBF files (or SQL Server, or Oracle, if you prefer), making it easy to run any kind of report even if the built-in reporting capabilities don't give you the results you need.
