Thank you for your help. I also found a sample Proforma invoice on the web (via google) that shows all the details.

>Do you mean "Pro forma" invoice perhaps? It is an invoice which is presented prior to the actual order being placed. It allows the recipient to show their accounts department that it is a valid expense that the person wants finance approval for. Then the order is placed and the actual invoice issued. The pro forma invoice can be the same as your normal invoice but with those words printed on it and it does not raise an entry in your books.
