Probably you can get a better answer from someone more experienced, but if you want to automate, you can use the Windows Update Agent, you can read about it here:

If I understood correctly (and remember correctly, I read about it like 8 months ago) your "Determine if a Windows patch is already installed" question would not require an answer, for it will be determined by this update agent
