could this be a factor? Could user's running on e: be
a factor? I will try your suggestion, but I don't
know how effective it will be since I am not experiencing

You should breakon the INSERT INTO line, if you then right click the title bar of the debugg window and choose Resume you should NOT break again, if you do you've found the line that';s creating the second record.
