Does the program run and disappear immediately? Do you have a READ EVENTS statement?
Something like:

* main.prg
do main_menu
do main_form
ON SHUTDOWN do MyOnShutdown
READ EVENTS

Do you have a SCREEN=OFF in CONFIG.FPW?
Or a _screen.visible=.F. anywhere in your program?
