Create a form, drop a grid on it with 5 columns and set their properties. Move code from Load, Init, Activate. Create new method SelectRecord. Invoke it from Select button. Also add Cancel and Close buttons. I hope it would work. The only problem might be with double click.

Also change ErrorMsg function to messagebox.
If it's not broken, fix it until it is.
