I never heard back from Stephen so I'm not sure, but I recommend the same thing for you...set a breakpoint in the constructor of the mmSecuritySetupForm to see what's going on. Just open your project in VS then click the Open File button and open the mmSecuritySetupForm.cs file and set a breakpoint.
