Is there an Extender Provider model to use for Infragistics menus or some other way to accomplish this? --something like mmMenuExtender?

You're right...we currently don't have an implementation for Web menu items...just the menu itself. You can use extender providers in ASP.NET although they are a bit different than Windows Forms. I'll add this to our "desired features list" but it will be quite a while before it's implemented.
