Have you searched for dlls that print or submit an HCFA1500 form (Either the form itself or an ECF Electronic Claim file)?

As an alternative, if you can find a legal pdf version of the form, you can use Acrobat to add the formfields and populate the fields on the form (and print it) from your VFP app.
