Unless the repair shop does financing (payment plans for repairs), you can leave the SSN off entirely or leave it in but make it optional. I would add:
HOME PHONE required
WORK PHONE required
email address optional (for service anniversary notices if the customer elects to receive them)
If you request the email address, a statement should be made about the security of the email adress guaranteeing that it will not be released to 3rd parties (if you can guarantee that).
Many businesses use email addresses to notify customers of upcoming sales or to remind the customer when a service is due or when an item if ready for pickup (if the customer wishes to receive such notifications via email)
