Normally Adobe Acrobat handles PDFs, thus a finished product. Would it be easier to produce either a VFP report (or a Word document) and finishing it into a PDF.

If I were doing this via Word I would create a standard survey form and merge the customer specific data such as Company Name, Address, Number of Employees.. Creating a document that contains the standard information with the customer specific.
