If you've got that many items, you're never showing all 40,000 of them anyways. One approach is to use 2 dropdown lists, where the first one contains a list of item Groups plus one more value that means "Show my current ticket items". When one of the first dropdown's values is selected, that filters the second's values.

If you have lots of items you might use even 3 or 4 levels of dropdown lists.
