By using Sergery's method, I can add a custom printer form in the printer spooler setup; however, the custom printer form is not shown when I select the paper size in the printers in the printer setting?

However, when I try the printer driver's properties, there is a tab called "User Customize" where I can add a custom form, then the form is shown up. It seems to me that the printer using a list other than the one used in printer spooler?!
