But, after the new PC was put into place I went for several months without it happening and I stopped doing this. I do remember that the common thing I did each time (I think) prior to this happening was modifying a table while the form was open.

I'm pretty sure I used to get in W98
It happens so rarely that I never kept a log of it. But you could be right as to amending a table at the time.
