How to define custom paper size?

I know I did it like 5-7 years ago but forgot how to do it. Now I became curious and am trying to figure out how to do it. When I go to my printer Properties page and look at the pull-down of paper sizes, I see a "custom" size, something that I entered there, specifying size of the paper (actually address labels). But for the life of my I can't remember how it is done.
