Your problem is the line:

ACTIVATE WINDOW screen

in the activate event of the form. If the form isn't a top level, it takes away the focus to the previous formular whenever you click in any of the textboxes. The form doesn't disappear, because it's always on top. You can't see that it's not active, because the title bar is not visible. If you uncomment the line, you can change text.
