Yes, it is possible. The most reliable way is to create a Windows Service that monitors the Event Log for new SQL Server login entries.

Also possible, but less reliable, is to examine the results of sp_who/sp_who2 in a DTS package that is scheduled to run every 2 or 3 seconds for a new connection. But you may miss a connection that opens and closes between queries.
