I had this when it was a crash. But, the last one was a clean reboot.

>I'm not 100% sure why this happens, I think somehow the changes are either not committed until you do a normal shutdown, or else your Active Desktop gets corrupted in a crash and icons get restored from a cache. But, all of this might happen only if you had a crash - if you did a normal shutdown or restart it should not happen.

Some of those icons have been updated since a while and several reboots were done since then. I was a surprise to me to see all those old versions coming back on the desktop.
