I understand your concern, that is also mine, but I wonder if it would work nonetheless. Checking the XP Resource kit, if you disable the short name generation, it seems that having 2 or 4 million files in a folder should not be a problem, the recommendation is to have the short-file name generation disabled if you have more than 300.000 files in a folder:
