I accidentally got an anonymous one from Kroger. My old one was just worn out, and I asked for a new one. The cachier gave it to me - after swiping it. Ah, so there's no registration at all. I just asked whether I could fill the form at home, having forgot my reading glasses, and sure got the card and the form. Of course, I never filled the form :).

Now of course they have the card number within the same transaction as my debit card number, and putting them together is trivial. It's just a matter of them wanting or not wanting to do that.
