Try putting a . (yes a .) in the box. This indicates the default SQL Server instance. Does this help?
Also try to run this from a dos command line.
osql /E /q "Select * from northwind.dbo.customers"
Does it return any results?
Once its finished use the exit command to get back to the command line.
