I would reccommend a program called Visual forms/PCL.

I have been using this program from before its windows days. The windows program is excellent. I have been using the program to overlay signatures onto reports but it was originally designed to design forms. You get a WYSIWYG screen to layout your form and the form can be saved as a PCL that FoxPro can copy to the printer before you run the report.
