This is just a "test" form that is not ready for production because it won't work on the Win2003 box. This one is on the NT4 box. Filling out the form, with "TEST" as the school name (to alert Customer service that I'm testing the operation) and filling in the rest of the required fields with meaningless information and clicking on "send" has been working for me.

What was the error that you received at post time? Did you fill in all the required fields? If the send is successful you are redirected to page "http://www.memorybook.com/index.htm". Did this happen?
