When I was loading batches (and getting all these invalid seek offsets problems) I found several cases where SSN and DOB were the same, but patient name in our database didn't match the name in the stage tables and the difference was first letter of the last name.
In other words, we may have Ruth Gordon in stage and Ruth Dgordon in our database, which doesn't make any sense. So, I found 32 cases (117 were until I ran select distinct to eliminate dups) with this problem. My manager and other colleagues were trying to track the problem. In one particular case they found a record in Trans_Events (actions history) that investigator changed the name. They didn't check other cases and I didn't try to track them either.
It is a possibility that insurances information comes with different last names and therefore our users change names deliberately. As I said, the new names which are in our database seem to be incorrect and don't match Client Notes.
If it's not broken, fix it until it is.
