Frank,
I stored the results as an XML (all answers in a single memo).
Not going into details because as a whole it's somewhat complex (but I did make it into a class and now for me easy).

>>>Hi,
>>>
>>>I need to design a system whereby the user can define certain questions and possible answers, then my code will display the data entry screen for these questions. It is basically a survey type system.
>>>
>>>The specs as I have understood them are:
>>>
>>>Unlimited number of questions are possible.
>>>
>>>Each question will consist of a string of text that gets displayed for the data entry clerk.
>>>
>>>Each question will have answers that are either checkboxes or text.
>>>
>>>For example:
>>>
>>>Question 1:
>>>Did you purchase any of the following online?
>>>
>>>Airline tickets: Yes[] No[] Web address:...........
>>>Hotel Accommodation: Yes[] No[] Web address:...........
>>>Other (please specify below): Yes[] No[] Web address:...........
>>>.......................
>>>
>>>
>>>Question 2:
>>>Compared to what you expected, kindly rate the following aspects of your visit.
>>>
>>>...................Excellent Good Average Poor Very Poor Not Applicable
>>> 1 2 3 4 5 6
>>>Airline Connection [] [] [] [] [] []
>>>Accommodation [] [] [] [] [] []
>>>Tours & Attractions [] [] [] [] [] []
>>>Beaches [] [] [] [] [] []
>>>
>>>
>>>
>>>Anybody done anything like this before and have any advice?
>>>
>>>Thanks,
>>
>>Frank,
>>I have done something like that and what I did was:
>>Prompt for question and answer type (checkbox list, radio, scaled, free form entry, answer required or not etc) along with preset answers. Generate an HTML from QA as a template.
