I did find my issue - which to me is pretty bizarre. I had a line
'SET PROCEDURE TO' in my INIT that was causing the second instance from firing. The error I was receiving was 'CANNOT CLEAR OBJECT IN USE'

I took that line of code out and it works fine. How do you call a SetComplete on the COM+ object that you were talking about?
