I think I may have found it!

Under Tools/Options..., under Query Execution from the TreeView, drill down to General and check the checkbox for "By default, open new queries in SQLCMD mode". That seems to allow the "New Query" button to work without prompting the connection.

Update! Still doesn't work for open file, though. :(
