You may have heard of the 1st British VC to be awarded to a living person for 40 years:

The last VC, as cited in the same article, was famously given to Col H Jones in the Falklands war. I believe that his was an act of foolhardy romantic recklessness, storming a MG nest Sgt Rock-style, leaving his men leaderless. Of course, I don't know the full facts but I doubt they were being picked off one by one, and his was the only recourse available.
