Don't know, Jim. But it seems to happen regularly. Twice today I've responded to someon'e answer to someone else; in both cases I thought the 3rd party would be interested in the reply and in both cases he was not on the cc list.
Try it yourself - you don't even have to save the reply - just look at the cc list.
