Hi,

I made a brief attempt at pulling your listing into a project but didn't have time to make the mods that would be required to get it running on its own.

Maybe you can narrow the problem down to either the MIDI parent or the child form by attempting to merge a simple child (along the lines of the child form in your original post)?

I also noticed that one of the links you gave refers to the 1.1 MenuItem rather than the 2.0 ToolStripMenuItem. I never used the older stuff so I don't know the differences but I'd be wary of confusing the documentation for the two types.
