Jake,
People certainly come from all over the world to attend one of the centers of excellence in the US. Suffice to say that the hotels in Rochester, Minnesota are always full- not just with foreign heads of state, but with wealthy out-of-staters as well.
But there are centers of excellence elsewhere too. If I needed revision of a cemented total hip, I think I'd go to the Princess Elizabeth Orthopaedic Centre in Devon, UK, or at least to somebody who trained there.
"... They ne'er cared for us
yet: suffer us to famish, and their store-houses
crammed with grain; make edicts for usury, to
support usurers; repeal daily any wholesome act
established against the rich, and provide more
piercing statutes daily, to chain up and restrain
the poor. If the wars eat us not up, they will; and
there's all the love they bear us."
-- Shakespeare: Coriolanus, Act 1, scene 1
