Guys!

From an Asian business standpoint, we're becoming substantially bigger as a small scale software dev company. From the original 8 persons, we are now 30 with 8 fulltime programmers. Others are technical support and testers.
My problem is that how to make our documentation globally standard? We're using VFP and C#.Net as our development tools. 3 persons are doing VFP apps for maintenance and 5 persons .NET programming. We've bought Team Foundation Server and I think the way we document our development processes is not yet globally standard. What I know of in documenting our development processes is really very simple. I use word in writing all the specifications of a module and projects.

Can you recommend a book that is designed specifically for this purpose. I know some books discussing documentations but I am not satisfied with the contents. Most of them are still incomplete to me.

If a software is enterprise wide in scope, hospital system with over a hundred users, how do you document the processes? What should be expected in the document before giving it a programmer and to make it globally standard?
