If you build another formset with 3 forms and a toolbar, does that work?

I would go to a backup of this formset if you can't get it to run anywhere. Without knowing what cuased it (the memory leak another reply mentioned maybe?) it seems to be corrupt now. You can always try opening the scx as a table and packing if you have no backup, I suppose.
