I have a small 4 row set of fields with a column lable over it describing the type of data "Hotels" to the left of each field is "Pref. 1" ,"Pref. 2", etc... and in a second column titled "Booked" I have 4 check boxes without any caption. When the user picks the hotel he/she wants from the ones available, they just click a check box. They can click more than one if desired. This type of form does not need a caption with the check box because the column heading describes with they are for. This is not a grid, just 4 textboxes and 4 check boxes.

It would be nice if the user tabs thru the check boxes then were easier to tell that it has the focus.
