The worst I've seen so far in Fox (before FoxPro even), from a guy obviously coming from mainframe:

case n=10
   @10, 2 say "Quantity:"
   @10, 15 get m.Quantity
   Read
   n=n+1
Case n=11
   @10, 2 say ...
   Read
   n=n+1
... etc etc

I kept a copy of the code somewhere... probably left it home on some unreadable 5.25 floppy. We'd need a museum for such exemplars.
