Could the problem be that it is up to employer's to provide for health care costs in this country?

Historically, the business environment allowed it because the US manufacturing environment could absorb it.

One key distinction between Europe and the US was that the ratio of resources to labor. In the US, labor was relatively scarcer.

That's changing and businesses in this country have to abosorb costs they do not elsewhere.
