Should they come up with original ways to shake hands, or kiss, as well? To me the point was made without worry that it had been made before in some other movie.

I believe it was also a reference to an earlier Sopranos episode in which Tony dreams he is losing his teeth one by one. The one where Annette Bening guest starred. At the time there was a terrific chat group at hbo.com/sopranos (since overrun by AOL-ers) and a regular who was a psychologist talked about what the fear of losing teeth meant.
