Lost is past tense, should be did you lose...

I knew it, but somehow I was not sure about lose being present tense when I was writing it and then didn't have time to correct.

Thanks for catching it, though. Much appreciated.

And also I guess I should say details of the form, not ON.
If it's not broken, fix it until it is.
