I use a few simple rules, outlined in the article I wrote for Universal Thread Magazine, August 2002.
This can be formalized with database design tools (CASE), such as the freeware InfoModeler / VisioModeler (Universal Thread Magazine, May 2006). I find it an interesting tools, but personally, I just use the rules outlined in the August article, and some common sense.
