The fielder just has to make sure he knows where the runners are when he catches that foul. He also has to make sure his feet are in foul territory (just in case he gets a case of butterfingers).

Actually, if the fielder drops the ball, or it touches him in any way and hits the ground, it's now a fair ball and in play.

Ground rules. There could just as easily be a chalk line that everyone agrees that if the ball passes this point on the fly it's a home run. That's why the ball can hit in the outfield, bounce over the fence and it's a "ground-rule double" and not a home run
