You can do one of two things:
1) Your form can be a VFP ActiveDoc, in which case the VFP runtime will need to be installed on every workstation. You can then have a separate COM control in the middle to do the data query and validation

2) Have an HTML front end that passes the info to IIS, then on to a middle-tier COM object.
