Two ways:
1. Tools->Options->Forms
Click on "Template Class" check box and select you classlib and class. Don;t forget to click on Set as Default button.
That way ALL new forms will be based on your Class.
2. Create a normal form. Then open it with Class Browser and click on "Redefine" button. Select your base class.
