Haven't used it in anger but I spent some time playing with ClickOnce deployment. It's not for everyone (or everything) but it does offer some interesting options. We tested one configuration that installed support files (e.g. .Net Framework, SQLExpress) directly from the MS website if required. Also automatic updates (and, sometimes more usefully, rollbacks :-} ) can easily be incorporated. I think there's a lot more configurable stuff there than most realize.
