But that doesn't always work out. Sometimes, you keep learning things along the way and you don't have time to go back and restructure to do it the way you would have done it with all the info up front.
Also, sometimes, even when you get it right the first time (based on the initial requirements), the thing grows over time (with new requirements) until it becomes a mess. Refactoring can be a way to turn that mess into something more manageable.
