Manually. The IDE doesn't suddenly to insert them under certain circumstances, if that's what you mean. You can manually enter the directive, or you can select a region of code, right-click, select Embed Selection, then #region..#endregion - there may be some VS keyboard shortcut for that, and I know there are some plug-ins that make it a bit easier.
