The heading for a field is assigned at the time you create the report. Since it's possible to change the heading for a particular field in a particular report, we can't go back and change the headings in the report after you change them in the data dictionary, as that would potentially overwrite your custom changes.

It sounds like the text was cut off at the right edge for some reason. The only time I've seen that is with a weird printer driver.

Once you've turned it into an advanced layout report, you can't use Step 2 to add or remove fields, because you've "fixed" the layout of the report in the Advanced Report Designer. However, you can easily add and remove fields in the Advanced Report Designer or make any other changes you wish.
