We use a Minolta and other that are sending scanned documents are using Cannon scanners or pdf generators. XFRX does a nice job of inserting pdf reports it creates into existing existing PDF documents. The biggest thing you need to stay away is linearized pdfs or page tree structures.

Our biggest problem has been linearized pdf as some of the audit companies sending us their audit workpapers via PDF are also creating them for the web so they linearize the report as it renders faster in their web system.

The best way is just to get the companies to give you a pdf created by their scanner and test with it. I spend the last couple weeks working with a few audit companies and we finally reach an agreement on the PDFs files they would send. Not all could comply so we went another way with them.
