open toolbox,
Right click in toolbox
Select `Choose items...`
Then rather then selecting from the list of stock items click on the `Browse...` command button.

Find the dll of your tool.
once you have found the dll click ok.
Visual Studio will verify the type library etc...

then
Your custom tool is added to the toolbox.
