>I've seen something similar a while ago, it was a registry problem. there was a fairly easy fix, too - download the Power Toys from Microsoft's Web site and install TWeakUI - it has a repair function that will fix icon mappings and colors (Control Panel/TweakUI/Repair; there's a combo box that will allow you to pick Rebuild Icons as an option.)

In Win 3.1, this usually meant your available resources (e.g., memory) were very low. Could be a similar situation in Win95 as well.
