Perhaps an idea... At least a starting point
I remember something from MS books : Windows Clipboard uses OLE technology, and OLE behaves differently when you cut/copy paste within an application or between two apps. Functions involved aren't the same.
Perhaps there was a DLL/EXE into your original Windows Dir (this onto you created the document) that contained a clipboard "misfunctionning function". I think : Win95 950a.
And, as Documents store temporary OLE information, the bug followed the Document when recording...
So you encountered the trouble onto several systems, as far as the document contains the error.
The error is perhaps an OLE pointer table (I don't remember the MS terminology) which wasn't cleared correctly the first time, or a bad object reference (from Registry).
So the document is corrupted.
What about this ?
